A suction irrigation instrument is a specialized medical device used in minimally invasive surgical procedures, particularly laparoscopy, to maintain a clear operative field by simultaneously removing fluids, debris, and smoke (from electrocautery) while delivering sterile solutions like saline for rinsing. These instruments are essential for enhancing visualization, reducing infection risks, and facilitating precise surgical maneuvers.

The instrument typically consists of a handle connected to suction and irrigation tubes, allowing the surgeon to control the flow rate and pressure of both functions. The design often features an integrated system with a sterile, single-use (disposable) or reusable construction. Disposable sets are increasingly popular as they eliminate the need for sterilization, reduce the risk of cross-contamination, and offer cost-effectiveness and convenience. Reusable sets are made from durable materials like stainless steel or autoclavable components for repeated use.

Suction irrigation instruments are available in various sizes and configurations to suit different procedures and anatomies. Standard sizes include 5mm and 3.5mm diameters, with working lengths of 330mm, 36cm, 45cm, and 340mm. They are classified by application, such as general laparoscopic sets for procedures like cholecystectomy, or specialized sets for urology, gynecology, and pediatric surgeries. Some models feature a smoke filter to manage surgical smoke, which helps maintain visibility and improve operating room efficiency.

Advanced designs include ergonomic features for improved handling, such as ambidextrous handles, protrusion designs for easy button distinction, and built-in sealing rings for air tightness. In robotic-assisted surgery, systems like the Remotely Operated Suction Irrigation System (ROSI) allow the surgeon at the console to directly control a flexible probe for suction and irrigation, increasing autonomy and precision. Other manufacturers offer customizable solutions with different diameters and lengths to meet specific surgical needs.
